Replace them with equivalents of `Span::{def_site,call_site}` from proc macro API.
The new API is much less error prone and doesn't rely on macros having default transparency.
This commit completely removes usage of the `panictry!` macro from
outside libsyntax. The macro causes parse errors to be fatal, so using
it in libsyntax_ext caused parse failures *within* a syntax extension to
be fatal, which is probably not intended.
Furthermore, this commit adds spans to diagnostics emitted by empty
extensions if they were missing, à la #56491.
Most notably this changes 'syntax::ext::base::get_single_str_from_tts'
to accept a trailing comma, and revises the documentation so that this
aspect is not surprising. I made this change under the understanding
that this crate is private rustc implementation detail (I hope this is
correct!). After reviewing all call sites, I believe the revised
semantics are closer to the intended spirit of the function.

This pull request refactors TokenTrees into their own file as src/libsyntax/tokenstream.rs, moving them out of src/libsyntax/ast.rs, in order to prepare for an accompanying TokenStream implementation (per RFC 1566).
