Remove `TypeAndMut` from `ty::RawPtr` variant, make it take `Ty` and `Mutability`
Pretty much mechanically converting `ty::RawPtr(ty::TypeAndMut { ty, mutbl })` to `ty::RawPtr(ty, mutbl)` and its fallout.
