Remove `ty_to_def_id`

fixes https://github.com/rust-lang/rust/issues/52341

The uses were mostly convenience and generally "too powerful" (would also have worked for types that weren't interesting at the use site)

README.md

The codegen crate contains the code to convert from MIR into LLVM IR, and then from LLVM IR into machine code. In general it contains code that runs towards the end of the compilation process.
